Although Geralt’s birthplace is unknown, he was certainly not born in Rivia. His mother left him and Vesemir took him in and brought him up in Kaer Morhen. It is in the realm of Kaedwen, where Geralt initially took Ciri, too. Now, his mentor Vesemir told him to gain a client’s trust as the Witchers must have a surname. And because he did not have anything else, Geralt adapted “of Rivia” to add to his name.

The mention of this specific part is available in the original book titled Baptism of Fire. Now, in their fight, they saved Queen Meve. She ruled Lyria and Rivia. So, after the battle, the Queen herself knighted Geralt and officially declared him as Geralt of Rivia.
